Financial Policy

Insurance
At Lifetime Eyecare Center we always
help patients maximize their insurance
coverage. If you have vision insurance,
then you probably have some coverage
towards routine eye exams and glasses or
contact lenses. If you have medical
insurance, then you have coverage
towards the treatment of eye disease or
infections. You may also have coverage
for routine vision exams through your
medical insurance. We will help you to
understand the difference between the
two and make sure you receive all the
coverage you deserve.
Financial Policy
The goal of Lifetime Eyecare Center of
Sleepy Eye is to provide the best
possible eye health and vision care with
high quality products.
We accept cash and checks as well as
Mastercard, Discover , VISA and debit
cards for your convenience.
If you do not have insurance coverage or
a high deductible plan, we offer a 20%
savings on a routine eye exam if
services are paid in full at the time of
service.
We wil bill your primary vision and
medical insurance carriers. If we are
not contracted with your insurance
company we will provide you with the
information needed to bill your
insurance/vision plan. In order to
submit to your insurance plan, you must
provide us with a current copy of your
insurance card at the time of service.
Lifetime Eyecare Center can not accept
responsibility for knowing your
insurance coverage, collecting from the
insurance company or negotiating
settlements on disputed claims.
You are ultimately responsible for your
account regardless of your insurance
coverage. Usual and customary fees will
be itemized for you and any estimated
fees will be due at the time of service.
A deposit is also required when ordering
materials and the balance due when
picked up or within 15 days. |
